Morning Erections - Nerve Damage, Vascular Damage?

Hi Dr.

I was a chronic masterbater for years. I never had any issues until a few years ago I started to get a mild case of ED (premature ejaculation, weaker erections).

Then about 10 month ago I noticed that I've started loosing sensitivity in my penis and I no longer was getting nocturnal or morning erections. I also developed a hard flaccid penis.

I can still get it up but I need constant stimulation to keep my penis erect. I was taking Alpha Blockers for an enlarged prostate and also took Viagra for about 9 months to help with the ED issues. I think it may have made things worse as I was getting morning erections until that point. Since then I've stopped taking both and just want things to go back to the way they were. I think I may have done some nerve damage and/or blood vessel damage.
What can I do to restore my morning erections?

If this is nerve damage, should I continue to stimulate the nerves in order to help them regenerate or stop sex and masturbating altogether?

I ask because It's been over 10 months and I'm afraid that if I don't stimulate the nerves the will loose the ability to regenerate.

Maybe you can explain what's going on with the nerves and or blood vessels and make a therapeutic recommendation and time frame for recovery?

The premature ejaculation and weaker erections were the warning signs of neurohormonal depletion that has been taking place. The continued practices had lead to a hypothalamic-pituitary-testicular lock which is usually considered the breaking point at which no nocturnal or spontaneous erections will be observed.

Viagra enhances the effect of Nitric Oxide released from nerve endings, however, in a long term it will cause much damage to the system and additionally deplete it from its NO levels for proper functioning. So there you have it - less prostaglandin E1 E3 and Nitric Oxide, as well as low androgens through the improper hypothalamic-pituitary-adrenal-testicular functioning, responsible for penile tissue health and integrity, will result in penile shrinkage and make ones member extremely prone to injuries (even through masturbation alone).

You should completely discontinue sexual activity for 2 to 4 weeks, and greatly diminish the activity afterwards.
