First off, thank you Dr. Richards for providing your services so freely.

I’m 52, married, not over weight. I am a testicular cancer survivor from 2002. I’m on TRT with low cortisol output throughout the day. My sexlife with my wife is basically zero since she hit menopause and my preoccupation with pornography. I’ve tried breaking from the porn many times without success. I have almost all the sexual exhaustion symptoms that you list. My ED is getting quite bad.

Once I find the courage to stop viewing pornography, will my libido go to zero? Presently I’m experiencing what probably is PSAS. I’ve been tested in the past with high norepinephine. Could my adrenal function return to somewhat normal if I quit masturbating? I currently am on hydrocortisone to be able to function.

Are there supplements that I can take address the PSAS so I can have a better chance of breaking free of pornography?

Any help would be greatly appreciated. Thanks very much.

ANSWER
Yes, usually TRT regulates Corticotropin-releasing hormone (CRH) stimulated hypothalamic-pituitary-adrenal axis activity in men, with divergent effects on Adrenocorticotropic hormone (ACTH) and cortisol, frequently leading to peripheral (adrenal) locus for suppressive effects on cortisol.

Your self-diagnosis is probably on the spot, I’d conclude that once you discontinue the pornography and masturbation practices the libido will go to practically non existent for 1-3 weeks at least.

“Could my adrenal function return to somewhat normal if I quit masturbating?”

Yes, however, it will take a good 4 to 12 month period. And you shouldn’t completely discontinue the practice.

“Are there supplements that I can take address the PSAS so I can have a better chance of breaking free of pornography?”

It’s a vicious cycle. However, the most important thing is to break the cycle of excessive inflammatory response keeping you in a state of “heat” and allow nervous (and not only) rejuvenation. The best way would be to improve the GABA-Serotonin nervous modulation on inflammatory response as well as HGH release, improve nervous and endocrine functioning, aid hypothalamic-pituitary-adrenal-testicular axis proper functioning, increase androgens, increase elasticity prostaglandin E1 E3 and Nitric Oxide, alter numerous gene expressions, and eventually rewrite the negative neuroplasticity developed with 5HTP-Nettle (half the dosage), GRB6-GABA, Alpha-AGH, Multi-Alpha, Alpha-Amino and Ultra-Purified-FishOil.

5HTP-Nettle and GRB6-GABA are to be gradually discontinued (75%, 50%, 25% every week) after 2-3 months. The libido will improve but remain relatively low for the initial 2-3 month period, though.

Also, you may want to possibly consider solutions to increase the effectiveness of the remaining testicle, given that it is the case.

As I am female, I am not sure if you will be able to help me or not but I cannot resist asking. I am 20 years old and I have been suffering from what seems to be a PSAS/IC like syndrome for a little over a year now. I suffer from a constant sensation that feels like the urge to urinate or the feeling of being aroused from the time I wake up in the morning until the time I manage to fall asleep at night. Urologic and gynecologic evaluations have not turned up much rather than slightly off hormone levels and very minimal amounts of endometriosis. Treatment for these conditions does nothing to help. Immediately before the onset of this condition I suffered from a long case of walking pneumonia followed by a pretty severe bout of thyroiditis. It may or may not have been triggered by a UTI or my discontinuation of birth control. So far I have tried overactive bladder medicines, urinary analgesics, diet change, physical therapy, muscle relaxants, antidepressants, yeast medications, and had laparoscopic surgery for endometriosis among many other things I can’t even remember. Nothing has even made a dent in this sensation. I am wondering if you think that any of your supplements could help me, any advice would be greatly appreciated.
Thanks a lot

ANSWER
Hello,

It does sound like a severe case of PSAS.

Yes, suppressing the excessive inflammatory response as a result of the possibly altered neurotransmitter levels after the birth control pills discontinuation may improve your condition immensely.

Glad if I could help!

To answer your question:
-A combination of Alpha-Amino, Multi-Alpha, GRB6-GABA, 5HTP-Nettle, and Ultra-Purified-FishOil will eventually increase prostaglandin E1 E3 and Nitric Oxide production
You may consider consulting a physician. However, the practice is to treat only the underlying symptoms, frequently neglecting the root of the problems.

Dr. Richards,

I have been suffering from overmasterbation symptoms for a few years now. The most debilitating symptom is PSAS like symptoms. Through much research I have found that this is due to inflammation and specifically from too much Prostaglandin E2 production. After having blood work done several times, my testosterone and sex hormones are perfectly fine and actually pretty high for my age. (I am 24). Yet I still suffer from erectile dysfunction sometimes and I also always have the urge to orgasm and my testicles are constantly in pain. I know this is from inflammation probably pushing on nerves. This has been going on for a few years now. I also have other inflammation symptoms in my body like dermatitis in my hands, high blood pressure (although I am very active and in good physical shape), difficulty breathing and inflammation of my nasal passages and airway. All of these symptoms are virtually ruining my quality of life and I just want to get my normal healthy life back.

I have stopped masterbation and even orgasming itself for a few months at a time, yet this doesn’t seem to help my symptoms at all. I have also been supplementing with about 20 grams of omega-3 fatty acids (fish oil) a day to try to decrease prostaglandin e2 production yet this doesn’t seem to help either even after a few months. I also supplement 5-HTP to increase serotonin production everyday. I am desperate now. Is my inflammation so great that it needs something stronger? I have contemplated trying an oral steroid to see if that would help with the inflammation, although I am hesitant because I am aware of the dangerous side effects. I just need some answers and some help please. It is hard to live like this and don’t want to continue doing so. Any help you can give me would be greatly appreciated. Thank you

ANSWER
Yes, you are suffering from symptoms of sexual exhaustion.

There is a time frame, usually, during which one will suffer from excessive inflammatory response for abnormal prostaglandin E2, histamine, glutamate, epinephrine, norepinephrine, and cortisol levels, while his hypothalamic-pituitary-adrenal-testicular axis is still functioning properly for normal androgen hormones production. So one will suffer the negative effects of internal inflammation, that through individual gene expressions will take numerous possible manifestations, the most severe of which will be abnormal immune responses, artery constriction, and numerous inflammations of vital organs. Also, excessive prostaglandin E2 will exert an excitatory effect on the prostate, penile tissues, and brain, leaving you in heat for PSAS like symptoms. The cycle will be reinforced every time one engages in sexual activities and will continue until has completely ruined proper nervous and endocrine functioning and killed hypothalamic-pituitary-adrenal-testicular functioning for less androgens. Even then, one may suffer from PSAS but will be unable to physically achieve erection and will finally come to the conclusion that something’s wrong.

The inflammation is usually a self supporting condition through numerous mechanisms – negative neuroplasticity, artery constriction, overburdened liver functioning, low androgens (not in this case), which means that inflammation will promote more inflammation in a vicious cycle that has to be broken.

The good news is that the hypothalamic-pituitary-adrenal-testicular axis is not locked, so you may expect to recover more quickly than others who have killed their axis proper functioning.
Just random set of supplements won’t prove to be very effective since the cycle of inflammation has to be addressed properly to avoid any negative feedback reactions.

On top of the abstinence I would suggest: Zeolite-DA, Alpha-Amino, GRB6-GABA, 5HTP-Nettle, Multi-Alpha, and Ultra-Purified-FishOil to better nervous and endocrine functioning, stabilize acetylcholine, serotonin, dopamine, and GABA levels, aid the nervous modulation on excitatory and inflammatory response, detoxify and better proper liver functioning, increase androgen hormones, and increase prostaglandin E1 E3 and Nitric Oxide.

UPDATE
Dr. Richards,
I am not doing so well with the recovery. I know from all of the symptoms that I have sexual exhaustion, and I have been suffering from these symptoms for over 5 years now. I have stopped all sexual activities for over 2 months, began taking the supplements you suggested, and trying to rest as often as possible. I even have been consulting with a nutritionist who specializes in dealing with adrenal exhaustion cases. I haven’t had much progress in any of the symptoms. Could my case just be severe and take longer to see results or is this going to be permanent? I cannot live like this anymore. I must recover or really my life is not worth living. I thank you for your site and information and plead for any further guidance. If you could help me recover from this you would single-handedly be giving me my life back and you have no idea how great that would be for me. Thank you for your time.

ANSWER
About 5-10% of the sexual exhaustion cases do not recover completely. However, they are usually associated with many years of sexual exhaustion (more than 10), as well as chronic alcohol and drug abuse to permanently damage nervous and endocrine functioning.

It is quite unusual not to experience improvements. The ongoing inflammation, even with the problems addressed, may be indicative of a strong negative neuroplasticity developed and/or slow brain adaptive properties. To rule out this possibility, my suggestion is treatment continuation for 2 more months. External irritants present for severe autoimmune and allergic reactions is another viable possibility.

Please, keep me informed!

I started masturbating and watching porn a lot starting a few years ago and I notice the more I do it,I felt like I was getting weaker mentally and physically. I would get up the next morning with stiffness and pain in neck and chest, low back and spine pain, and my vision would be really blurry and a headache that would last for some days to a week. Is this all coming from masturbating too much? Do I need to stop masturbating for a while?

ANSWER
Yes, overactive sexual activities may and will cause every one of the listed symptoms.
You may want to go over this article.

Such activities will consume too much neurotransmitters and neurohormones and will leave your organism depleted, in an ongoing never ending cycle of inflammation.

Yes, you should discontinue masturbation practices for a period of time.

UPDATE
also I forgot to mention now I have ibs along with the feeling that I need to masturbate, its like my penis is constantly in heat and needs to ejaculate, is that also coming from too much masturbation

ANSWER
Excessive local levels of prostaglandin E2 may lead to these exact symptoms.

I will try to do this…forgive me if I don’t.

Firstly, I’ve been running around to Urologists/ER’s the past few months not even knowing what PSAS was. I am in NYC, and although I mentioned that I have a history of urine retention, and had a recent ileostomy (however, my rectum wasn’t removed)and all thought i was “nuts” when I told them I have constant sexual urges that come out of nowhere, don’t leave and only increase, with momentary relief if I do orgasm (since, I have been unable to achieve orgasm, and 6 days ago, I have to CATH every time i urinate, which is a nightmare, while excited. I was checked for Chlamydia, prostatitis, BPH…nothing, although I wonder now..as i feel a huge lump in pelvis.

Simulataneously, I had this ileostomy surgery almost a year ago…however, he didn’t remove my rectum and gave me a temporary “loop” ileostomy. I’ve complained to the surgeon about a severe pressure in sigmoid/deep rectum area 24/7 (can barely sit)…but he said, “not untilyou gain weight”. Unfortunately, I have a double herniated large intenstine, so when I get past 135 lbs (6’1″ tall), it feels as though my belly is ripping in two.

On my last visit…on the access-a-ride trip home, I became annoyingly stimulated. Since then, I have been waking up w/severe urges, I fear eating as i’m on BOOST Plus I mix w/Cranberry juice and soft foods…protein seems to really get me going. I stopped drinking milk years ago…and went on Soy, as I went on a diet a dropped from 267 to 179…then, I lost my ability to urinate and needed laxatives; developed “severe global colonic inertia’ (thus the ileostomy).

I will stop now, as I do not want to overwhelm you…but, let me just leave with this…I have never suffered like this in my life, and i fear awakening, I fear walking, because once an episode begins, it can go straight for 9 days, even w/ powerful narcotics for the pains from Avascular Necrotic Hips, knee and shoulder, Peri-Neuropathy and the double herniated intestine, and diverticulosis. Sometimes all the pains with the sudden addition of psas and cathing everytime I need to urinate, I want it all to end. And I’m 49 years w/a degree in Psychology.

If you have any suggestions for me, or would be willing to converse with my Doctor(s), please let me know. Unfortunately I’m on disability, but, I’ll do what I can.

PS…sice cox 2 is bad, and 1 & 3 are good…would adding Celebrex do anythng to relieve even a little bit of my episodes, as I Must travel to surgeon on Monday and try to convince him again to remove my colon/rectum and or fix my hernia.

ANSWER
Any kind of interventions in the pelvic area are very dangerous and can lead to numerous complications.

My guess is that the parasympathetic nerves were damaged or pinched. Pelvic Splanchnic Control is not working properly.

The bad news is that a mere fix in the biochemistry won’t deliver the results needed.

Cox 2 is not bad, it is vital for the proper functioning of the human body. However, everything in excess may prove to be harmful. Celebrex may bring some relief, but I doubt it will be a notable one.

Can you please tell me how one can gradually rejuvenate their neuro-endocrine function and boost your prostaglandin E-1/E-3 and Nitric Oxide production?

Is this done my simply not masterbating a for period of time?

ANSWER
If you have fully discharged your brain’s nervous modulation on the release of excessive prostaglandin E2, DHEA, testosterone, and DHT (for PSAS), and dopamine-norepinephrine-epinephrine conversion, it may prove to be difficult to get back on track.

UPDATE
I would like to try.

ANSWER
1. Discontinue any sexual activities for 1 to 3 months.
2. Take massages to increase the proper blood circulation.
3. Balance your diet.
4. Restore your nervous modulation on your inflammatory hormones release by taking the proper supplements: Multi-Alpha, 5HTP-Nettle, GRB6-GABA, Alpha-Amino, and Ultra-Purified-FishOil, this will eventually lower the stress hormones and boost yout Prostaglandins E1 E3 and NO for healing, that is unless you have your hypthalamic-pituitary-testicular axis completely locked.

Hi Dr. Richards–
I run the support groups (2) for women who suffer PGAD—and have it myself–can you tell us more in simple terms what this comment means? I would love to know what you think is a cause, what you think women could try for relief–I cannot tell you how horrific it is to live with this–

Can you contact me at pgadsupport@gmail.com? I found this site by my Google Alerts that come into my gmail account –anything flagged with PGAD will be sent to me–if there is anything to know we want to hear it please–

Sorry–I didn’t give my name–
Jeannie Allen, Founder of PSAS Support

Best Regards-
Jeannie Allen

ANSWER
Jeannie Allen,

I commend you for the works on the subject. It is always inspiring to help people.

Here is what I consider the problem:
In-between 14 to 60 (most frequently 28-34), the hypothalamic-pituitary-ovarian axis may start to produce less prolactin. In addition certain liver proteins may become low, thus an improper suppression of abnormal prostaglandin E2, oxytocin, estrogen, DHT, and testosterone levels may be observed. Basically, the persistent sexual arousal in women is a clitoral, vulva, uterine, and vaginal inflammation, and so as a direct response to the increased prostaglandin E2, oxytocin, testosterone, DHT, and epinephrine levels.

The reasons to why is this happening are numerous:

Masturbation, which will constantly induce excessive inflammatory hormones. Note that if the endocrine system and specific nervous modulations may very well fail to modulate the release if are exhausted.

Certain dairy products with a Prostaglandin E2 analog so lead to forms of PSAS through numerous gene expressions

Birth control pills

Pregnancy

Abnormal nervous modulation, due to certain nts. and hormonal lacks

The main cause for PSAS is physiological, basically every PSAS sufferer was “short” on proper inflammatory suppressive action by the nervous modulation, specifically Serotonin and GABA. Once a cycle of excitation and inflammations has begun, it will additionally drain the needed resources for the proper modulation. The obvious conclusion is that the cycle has to be broken.

Generally speaking, it can be done by doing the following things:

1.A complete abstinence for a period of time.
2.A balanced diet.
3.A gradual rejuvenation of the neuro-endocrine function and nervous modulation on inflammatory and exitatory hormones.

This will most probably get things under control.

Wish you all the best!

UPDATE
Thank you for all this info–I did click the links to the web site for the things you suggested.

When you say a complete abstinence (which is the hardest thing for sufferers to do due to the pressure and distraction) between abstinence and using the suggested nutritional components, do you have any suggested ball park idea of a time frame that we would start to feel an improvement (as in an decrease in the tingling sensations)?

I know everyone is different but are we talking a month? Six months? A year? What about L-Arginine and Glutamine for the deepest inflammation along with balanced diet?

Do you see sugar in any form, sodas, chocolate, boxed food products as increasing inflammation at all? Or diet sodas with aspartame which is poisonous on it’s own?

Thank you for being there–I have posted this link on the support groups –I have a feeling others will be coming here too–be prepared!

Best Regards-
Jeannie

ANSWER
Jaennie, It’s my pleasure.

Yes, everyone’s different, but speaking very roughly I would say that an improvement is to be observed in a month.
There shouldn’t be any need for additional L-arginine and Glutamine.

The refined sugar is harmful in every way. One can expect much problems for an already faulty endocrine system, and more inflammation as well. It’s always a good idea to avoid refined sugar altogether.

One should stay away from diet sodas as well, or anything suspicious in nature for that matter. It’s best to drink clean water.

Glad I’ve been helpful. My private practice consumes most of my time, but will make sure it won’t take me more than several days to respond properly to everyone.

UPDATE
One other thing please Doctor–

Regarding what you feel is causing PGAD, could you tell me what research evidence you have to base this on?
Can you provide us with some references to published articles in professional journals so that we might purchase and read them?

I think we should know this before we put out money for those items you listed.
Thank you–
Jeannie

ANSWER
Jeannie, of course. I plan to systematize the all references and publish them in a special article. Since they are more than 150 (the online sources) it will be time consuming, so I will create the ref. chapter in about 20 to 60 days.

UPDATE
I have suffered with PSAS off and on for 20 years. I started at age 28 after coming off the birth control pill and it becomes worse at ovulation time and around my period. I would like to try one of the supplements that you recommended–mainly the Seronex–but I don’t know if it is for women to use and if it might help me. I eat a fairly balanced diet but will definitely limit the sugar intake and the milk intake (unless it is organic). Please let me know what you think about the Seronex or other things I might try. Thank you.

UPDATE
I have suffered with PSAS off and on for about 20 years after coming of the BCP (I am 48 now). It is worst during ovulation and around my period (with vulvar swelling), but I still feel that the nerves in the genital area are sensitized most of the time–feeling prickly, tingly, achy, etc(even without swelling). I was wondering–do you recommend that women take all of the supplements that you suggest–especially the ones for men? One supplement stated that it will enhance sexual well-being. I want to suppress it more than enhance it. Also, should bloodwork be done before starting these supplements–ie testosterone, prolactin, prostaglandinE2 levels, etc. Do my symptoms fit into your theory for PSAS for women?

Another question–I find after sex, I don’t have full resolution for a long time–sometimes 8 hours. The area stays swollen. I only have sex about once per week–not excessive–and I don’t masturbate at all. Could this be related to hormone levels? I would appreciate any info you have. Thank you

ANSWER
Men are also suffering from altered neuroplasticity for additional excitatory and inflammation hormonal release to put their brains and internal organs in “heat” and it is actually what leads to sexual exhaustion later. The supplements will break the ill cycle by aiding the blown modulation on excitatory and inflammatory hormonal release. They will also be of great aid as far as altering the brain neuroplasticity and rewriting the reward circuits goes. It means that the products will kill the sexual desire if it is caused by the wrong factors (example excessively binded prostaglandin E2) and later improve testicular functioning, thus creating stronger sexual desire that is not so severely based on inflammatory responses. This is true and for women, the dreaded self-supporting cycle of misregulation of certain hormonal releases and sexual excitation will be broken by the aided nervous modulation for normal functioning. A healthy sexual desire will then occur.

UPDATE
Are the new products what you suggested previously, or something new? Also would it help to get bloodwork, or would that matter? I am at a point in my cycle now where my hormone levels are rising (heading towards ovulation) and the genital area becomes somewhat swollen and actually hurts with some burning. Should I have bloodwork done when I feel the worst ( which will probably be in a day or so)? I would appreciate any feedback you can give me. Thank you.

ANSWER
They include most of the active ingredients of the previous products, but are improved to provide even greater results.

A bloodwork in this case will be inconclusive. The burning and the slight pain is most probably caused by the excitatory and inflammatory hormone prostaglandin E2 present in the local tissues.

UPDATE
How long do these supplements need to be taken once they correct the problem? Would it come back if the supplements were stopped? Also, I am on a supplement called DIM that is an estrogen metabolizer. Would I need to come off of this? Thank you for all of your info.

ANSWER
Once the problem is corrected you should take them for at least 2 months to permanently change the negative neuroplasticity of excessive inflammatory and hormonal release that took place after the birth control pills discontinuation.

There should be no negative DIM interaction with the products. You may try and observe for yourself if DIM will prove to be of any particular assistance or not.
